Wenn ein schwerstkranker Mensch, der zuhause betreut wird, unter unertr\u00e4glichen Schmerzen, Atemnot, \u00dcbelkeit, Angst oder anderen schwerwiegenden Symptomen leidet, f\u00fchlen sich Angeh\u00f6rige schnell hilflos, \u00fcberlastet und reagieren oft panisch, wenn es dem Wunsch des Patienten entspricht, einen Krankenhausaufenthalt zu vermeiden. Bei solchen Krisensituationen oder anderen Notf\u00e4llen gen\u00fcgt ein Anruf beim Palliativteam Hochtaunus und ein Arzt und\/oder Pfleger kommt<\/strong>, um Hilfe zu leisten und Entscheidungen zu treffen.<\/p>\n

Das Palliativteam Hochtaunus, ein Team der Spezialisierten Ambulanten Palliativversorgung (SAPV)<\/strong>, unterst\u00fctzt rund um die Uhr Patienten mit einer fortgeschrittenen Erkrankung und komplexem Versorgungsbedarf sowie deren Angeh\u00f6rige. Ziel ist es, Symptome zu lindern, die Lebensqualit\u00e4t der Betroffenen und ihrer Familien zu erhalten<\/strong> oder zu verbessern sowie ihnen Entlastung und Sicherheit zu bieten. So soll der gew\u00fcnschte Verbleib des Patienten bis zu seinem Tod zuhause erm\u00f6glicht werden.<\/p>\n

Die Spezialisierte Ambulante Palliativversorgung (SAPV) durch das Palliativteam wird vom Hausarzt verordnet, die Kosten \u00fcbernimmt die Krankenkasse.<\/strong><\/p>\n

Das Palliativteam Hochtaunus betreut palliativmedizinisch Patienten, die unter einer fortschreitenden Tumorerkrankung, Lungenerkrankung, Herz-Kreislauf-Erkrankung oder neurologischen Erkrankung leiden.<\/p>\n

Das multiprofessionelle Palliativteam Hochtaunus, bestehend aus Palliativmedizinern, Palliative Care Fachkr\u00e4ften sowie speziell ausgebildeten Therapeuten, versorgt die Patienten zuhause, im Pflegeheim oder Hospiz mit den notwendigen palliativmedizinischen, pflegerischen und psychosozialen Ma\u00dfnahmen.
